Subject of the search
The SP-167 instructions require the subject’s consent and notarized signature in Section 1. Check the identifying information before signing.

Another person or agency receiving results
That recipient or agency representative must sign and have Section 2 notarized as applicable. Do not assume the subject’s signature completes the recipient’s section.

Mail-results and contact details
Use the intended recipient’s name and mailing address. Include contact information so a discrepancy can be resolved. Check that the address and the signer in the recipient section correspond to the intended delivery.

Generate, review and print
The VSP form generator produces paperwork for mailing; generating it online does not complete the mail-in request. Keep a copy of the completed paperwork for your reference.

Send the required payment and signed form
Follow the generated form’s payment instructions and mail the completed packet to VSP CARE, 7700 Midlothian Turnpike, North Chesterfield, VA 23235. Submit one form per request.

Allow processing and delivery time
VSP says it offers no same-day or expedited service. Its guidance describes approximately 15 business days after receipt, with the FAQ also giving a 15–20-business-day range. Account for postal delivery separately when arranging a deadline.
